Terms and Conditions
Responsibilities:

Attend scheduled appointment consistently

Attend all scheduled sessions

Arrive on time
-
Participate actively in all aspects of the counseling session: asking questions, completing assignments.

Cancel no less than 24 hours in advance if unable to attend a scheduled session

Missed sessions without prior notification will be considered a "No Show" and will forfeit prepaid fee associated with the session or will be billed as a regular session, and the rescheduled session will be charged with the next invoice

3 "No Show" or repeated cancellations may be subject to suspended or discontinued services

Be open and prepare sessions by determining what needs to be discussed during the session
Equilibrium Counseling agrees to work cooperatively with the parents giving consent for their child to participate in its program and the patient (adult or minor) giving assent to his/her participation in order to achieve a successful counseling session. All parties will fulfill their responsibilities to help to the best of their abilities. I understand that this contract may be suspended or discontinued for breach of contract.

By signing this document, I acknowledge that if anyone is hurt, or property, damage, and/or stolen outside of our office during my participation/ my child participate in a program, a court of law may find me to have waived my right to maintain a lawsuit against Equilibrium Counseling or its employee on the basis of any claim of which I have released them herein.
